Self-assessment questionnaire (eASSIST Screening Tool)
The eASSIST is an electronic version of the Alcohol, Smoking and Substance Involvement Screening Test (ASSIST) which was developed by the World Health Organisation. The ASSIST has eight questions and take approximately 5-10 minutes to complete. The ASSIST helps identify the risks associated with substance use and the personalised feedback helps explore options for change.

From your answers today it is recommended that you take action and speak to your healthcare provider or a drug and alcohol service about your alcohol and or other drug use. Whatever option you choose to take, know that there are non-judgemental and supportive services ready to assist you.
