Northern Connect is a program that supports people with mental health and alcohol and/or drug (AOD) issues.

This free service provides psychological support to address both mental health and AOD problems and can provide care coordination to ensure your needs are addressed by the most appropriate services in the community.

The service is delivered in partnership by Sonder, Life Without Barriers, Uniting Communities & Drug and Alcohol Services SA, working together with you on your goals.

This program can support you by addressing the practical, physical, social and emotional issues that often accompany living with mental health and AOD issues.

To make a referral into the program you can:

- Call our office on (08) 8209 0700 or

- Have your GP complete a Mental Health Treatment Plan and fax to (08) 8252 9433 or

- Other service provider complete and fax (08 8252 9433) through a referral form. Referral form can be found on the Sonder website (www.sonder.net.au) or by calling us on (08) 8209 0700.
